This paper provides an analysis of nursing as a knowledgeable discipline. We examined ways in which knowledge operates in the practice of home care nursing and explored how knowledge might be fruitfully understood within the ambiguous spaces and competing temporalities characterizing contemporary healthcare services. Two popular metaphors of knowledge in nursing practice were identified and critically examined; evidence-based practice and the nurse as an intuitive worker. Pointing to faults in these conceptualizations, we suggest a different way of conceptualizing the relationship (...) between knowledge and practice, namely practice as being activated by contextualized knowledge. This conceptualization is captured in an understanding of the intelligent creation of context by the nurse for nursing practice to be ethical and effective. (shrink)

The concept of professional autonomy has figured prominently in literature that addresses nursing's project of professionalization. Nursing's capacity to determine the nature and scope of its practice is related in important ways to the location of practice. Within highly structured environments such as acute‐care hospitals, nurses' professional autonomy has frequently been contested yet is often implicated by nursing's elite as a necessary condition in the construction of quality work environments. Professional concerns and management practices related to retaining experienced nurses to (...) support sustainability in healthcare delivery systems' impact on the ability of nurses to practice autonomously. Our paper focuses on the emerging field of practice of chronic disease management. We describe the complex relationships negotiated by a nurse in a theoretically autonomous practice setting as she seeks to fulfil both the requirements of a research protocol designed by physician experts representing the specialty of renal medicine, and her professional obligations to respond to the expressed needs of patients with early‐stage renal disease. We utilize a case study approach to explore particular contemporary concerns that nurses in practice confront as they attempt to accomplish professional relationships with patients central to achieving prescribed medical outcomes where nursing practice, as an element of the achievement of those outcomes, is constituted as absent or unacknowledged by the medical researchers leading the project. Implications for nursing's discourses on the professional project of autonomy will be discussed. (shrink)

Both traditional and recent accounts of the beneficial and therapeutic effects of psychedelic experiences tie these effects to specifically epistemic changes, for example the enabling of spiritual or psychological insight, or disruption of problematic beliefs or thought patterns. While these alleged benefits have sometimes been thought to be facilitated by false or even delusional beliefs (e.g. Pollan 2015), recent philosophical discussion strikes a more optimistic tone, arguing that the epistemic risks involved with psychedelic drug use tend to be relatively benign (...) and outweighed by epistemic benefits (Letheby 2021). In this paper, we seek to nuance this picture by drawing attention to the crucial role played by social factors in determining the epistemic effects of psychedelic experiences. We argue that the very openness of mind and heightened sense of clarity and certainty that is often thought to facilitate the epistemic benefits of psychedelic drug use, also make the user more vulnerable to adverse epistemic influence from the social environment, to a degree not acknowledged in the current literature. Examining the epistemic influences of the social environment is thus important for understanding the necessary precautions of informed consent and overall safe drug use. (shrink)

Higher education institutes both foster the advancement of knowledge and address society's socioeconomic and environmental challenges. To fulfil these multiple missions requires significant changes to how the role of a researcher is perceived e.g. a researcher identity that is congruent with the objective of contributing to fundamental knowledge while also engaging with non-academic actors, broadly, and entrepreneurship, in particular. We argue that the early stages of an academic career—namely the PhD training trajectory—and the knowledge networks formed during this period have (...) a major influence on the scientist’s future capacity to develop an appropriate researcher role identity. We draw on knowledge network and identity theories to investigate how the knowledge networks (i.e. business, scientific and career knowledge networks) of PhD students promote changes to, reinforce or conflict with the perception of a researcher role identity. Our longitudinal qualitative network study includes PhD students and their supervisors funded by the H2020 FINESSE project. At the network level, we show that scientific knowledge is distributed equally throughout young academics’ networks but that entrepreneurial (business) and career knowledge tend to be concentrated around certain individuals in these networks. On the PhD student level, we observe different pronunciations of the researcher role identity linked to students’ interactions with their knowledge networks. We distinguish identity conflicts due to misalignment between ego and alters which leads to withdrawal from the network. Our findings have practical implications and suggest that universities and PhD student supervisors should support PhD students to develop a researcher identity which is in line with the individual PhD student’s expectations. Background Several studies suggest that deviations from proper authorship practices are commonplace in medicine. The aim of this study was to explore experiences of and attitudes towards the handling of authorship in PhD theses at medical faculties in Denmark, Norway, and Sweden. Methods Those who defended their PhD thesis at a medical faculty in Scandinavia during the second half of 2020 were offered, by e-mail, to participate in an online survey. Survey questions dealt with experiences of violations of the first (...) three of the ICMJE authorship criteria and misuse of authorship order in the thesis articles, as well as respondents’ attitudes to these matters. Both questions with fixed response alternatives and questions with free-text responses were used. Quantitative data were analysed statistically using the Table functions in SPSS 25 and Chi-2 tests. Free-text responses were analysed qualitatively using manifest content analysis. Results 287 valid questionnaires were returned (response rate: 34.1%). Almost half (46.0%) of the respondents reported that the ICMJE authorship criteria were not fully respected in at least one of the papers in their thesis, while a vast majority (96.7%) found it important that authorship is handled according to the ICMJE authorship criteria. 24.4% reported inadequate handling of authorship order in at least one paper. The qualitative results provide a wide spectrum of examples of how the ICMJE authorship criteria are circumvented. Conclusion Despite increasing educational efforts to reduce deviations from good research practice at Scandinavian universities, the handling of authorship in medical papers remains problematic. La recherche conduite dans le cadre de la thèse de doctorat (CROUZIER 2003) a été focalisée sur l’analyse des liens professionnels tissés entre les dispositifs RASED et les équipes éducatives des écoles primaires. Nous avons co-construit les données nécessaires à leur compréhension et choisi des traitements combinant les avantages du quantitatif et du qualitatif. Nous avons en particulier retenu une approche statistique fondée sur l’analyse statistique textuelle (LEBART SALEM 1994) pour explorer le corpus construit à partir d’entretiens auprès d’un échantillon (...) de professionnels concernés par les aides spécialisées à l’école primaire. Le choix de l’approche statistique s’inscrit dans une conception du raisonnement statistique développée par J.C. Régnier (REGNIER 1998, 2002). L’opérationnalisation du traitement a été rendue possible par le recours à un outil informatique, le logiciel S.P.A.D. (Système Portable de l’Analyse des Données) du CISIA. Cet outil permet de décrypter les corpus obtenus par des méthodes d’analyse factorielle de correspondances multiples. Différentes procédures ont été appliquées aux corpus bruts puis codés. Cette approche “quantiqualitative”, apportant une plus-value interprétative, décrypte de façon singulière la complexité de la collaboration entre acteurs. Abstrac: This PhD dissertation (CROUZIER 2003) focuses on the analysis of the professional relationships between the French institutional comprehensive remedial scheme known as RASED and educational teams in primary schools. We have jointly identified the data relevant to a thorough understanding of such relationships, opting for a method of data processing combining the advantages of both quantitative and qualitative approaches. We have chiefly relied on a specific brand of textual statistics (LEBART SALEM 1994) in order to explore a corpus made up of interviews with the primary school staffs involved in the scheme. The statistical approach accords with the conception of statistical processing developed by JC Regnier (REGNIER 1998, 2002). The processing itself has been engineered thanks to CISIA’s software, known as SPAD (portable data processing system). It has enabled us to decrypt the different types of corpuses through Correspondence Analysis (CA). Both the raw and the coded corpuses have been submitted to different types of processing. The approach, combining quantitative and qualitative elements, opens up on to new powerful interpretative insights and sheds a new light on the intricate network of interaction between the people involved. Starting with a specially written introduction in which Mary gives an overview of her career and contextualises her selection, the chapters are divided into three parts: Educational Assessment and Learning Educational Evaluation and Curriculum Development Educational Research and the Improvement of Practice Through this book, readers can follow the different strands that Mary James has researched and written about over the last three decades, and clearly see her important contribution to the field of education. This introduction offers a brief survey of the very notion of hyperintensionality followed by a summary of each of the papers in this collection. The papers are foundational studies of hyperintensionality accompanied by ample philosophical applications.Hyperintensionality concerns the individuation of non-extensional entities such as propositions and properties, relations-in-intension and individual roles, as well as, for instance, proofs and judgments and computational procedures, in case these do not reduce to any of (...) the former. Hyperintensional individuation is frequently also referred to as ‘fine-grained’ or sometimes simply ‘intensional’ individuation, when ‘intensional’ is not understood in the specific sense of possible-world semantics or in the pejorative sense of flouting various logical rules of extensional logic. Background: Clinical investigation is a growing field employing increasing numbers of nurses. This has created a new specialty practice defined by aspects unique to nursing in a clinical research context: the objectives (to implement research protocols and advance science), setting (research facilities), and nature of the nurse–participant relationship. The clinical research nurse role may give rise to feelings of ethical conflict between aspects of protocol implementation and the duty of patient advocacy, a primary nursing responsibility. Little is known about whether (...) research nurses experience unique ethical challenges distinct from those experienced by nurses in traditional patient-care settings. Research objectives: The purpose of the study was to describe the nature of ethical challenges experienced by clinical research nurses within the context of their practice. Research design: The study utilized a qualitative descriptive design with individual interviews. Participants and research context: Participating nurses (N = 12) self-identified as having experienced ethical challenges during screening. The majority were Caucasian (90%), female (83%), and worked in outpatient settings (67%). Approximately 50% had > 10 years of research experience. Ethical considerations: The human subjects review board approved the study. Written informed consent was obtained. Findings: Predominant themes were revealed: (1) the inability to provide a probable good, or/do no harm, and (2) dual obligations (identity as a nurse vs a research nurse). The following patterns and subthemes emerged: conflicted allegiances between protocol implementation, needs of the participant, desire to advance science, and tension between the nurse–patient therapeutic relationship versus the research relationship. Discussion: Participants described ethical challenges specific to the research role. The issues are central to the nurse–participant relationship, patient advocacy, the nurse’s role in implementing protocols, and/or advancing science. Conclusion: Ethical challenges related to the specialized role of clinical research nurses were identified. More research is warranted to fully understand their nature and frequency and to identify support systems for resolution. (shrink)

Few, if any, present‐day undergraduate degree courses in Theology include in their syllabus a study of the Epistle to the Hebrews or other New Testament writings other than the Gospels and the Pauline epistles. The result is in effect that we create a canon within a canon.This paper, originally read at a postgraduate seminar, gives reasons why Hebrews in particular should not be neglected.Hebrews provides evidence of the diversity of early Christian tradition, for example, with its teaching that it is (...) impossible to be re‐admitted to the community of faith, having once abandoned it, and with its unique use of Israel’s day of Atonement rites in its presentation of Christ. Moreover, the very genre of Hebrews merits particular interest.Hebrews also evidences a Christian community which has yet to break with Judaism. Its thoroughly Jewish background illustrates for students of the New Testament the necessity of knowing the Jewish Scriptures as well as the writings of the New Testament. Moreover, a study of the Epistle could make a constructive contribution to present‐day Jewish–Christian dialogue, even if in the past it has been enlisted on the side of a thinly‐disguised anti‐Semitism.Finally, Hebrews brings the student face to face with the metaphorical character of much of the language of the New Testament – a form of language which is not to be taken less seriously than other kinds of language; and in this case, Hebrews’ Day of Atonement metaphors issue in new insights – in an innovative theology of access to God.For this and other reasons, the study of Hebrews has an important contribution to make to theology degree syllabuses. Current controversies about the ordination of women have shown the need for a re‐examination of what the Christian Church means by priesthood. This article looks at the Epistle to the Hebrews’ contribution to our understanding. To that end it focuses on the institution of priesthood in its first‐century Jewish context and shows the use made of it by the author of Hebrews in his presentation of Christian faith.Section 1 emphasizes some all‐important differences between the NT’s use of the language of (...) priesthood and ours. Not least, it nowhere uses “priest” to designate Christian ministers. All the more striking, therefore, is Hebrews’ depiction of Jesus as “high priest”.Section 2 discusses Judaism’ Day of Atonement ceremonies – Hebrews’ dominant cultic model. In the comparison drawn between Christ’s death and exaltation with these rites, he becomes not only the high priest but also the expiatory victim.As far as Judaism’ cultic institutions are concerned, however, Jesus was not and never could be a priest, since he was of the tribe of Judah rather than Levi. Hence Hebrews appeals to Melchizedek. How this non‐Israelite model is used by Hebrews to subvert the whole notion of priesthood as caste is discussed in section 3.Finally, in section 4 Isaacs concludes that for Hebrews there is no longer a role for an ongoing priesthood, since Jesus has definitively achieved access to God, which was its raison d’être. Melchizedekian high priesthood is unique to Christ; neither inherited nor transmitted. Hence, unlike other NT authors, for Hebrews even the church as a corporate body is not a priesthood. As its closing chapters show, in this Epistle the cultic model gives way to the more inclusive one of pilgrimage. (shrink)

Mary Daly had a complicated relationship to the Catholic tradition. While it is commonly assumed that she rejected it thoroughly, this article offers a more nuanced look at the various ways in which it shaped her thinking. What is clear is that she had a decisive impact on the Catholic tradition, indeed on religion in general. Language about the divine, images of deities, human participation in things spiritual will never be the same after her thorough-going feminist critique. This is a critical look at the question of design from a feminist theological perspective. The author analyzes James Moore's 1995 Zygon article, “Cosmology and Theology: The Reemergence of Patriarchy.” Then she looks at the relationship between science and religion from a feminist perspective, focusing on the kyriarchal nature of theology itself in light of the myriad power issues at hand. Finally, she suggests that, instead of pondering the notion of design, scientists and theologians might more fruitfully look for new (...) ground for dialogue since feminist scholars are asking very different questions, not just answering questions differently. This paper explores the continuing relevance of Karl Rahner’s work on development of doctrine to a church within a world marked by an emerging postmodern consciousness. It focuses primarily on three elements of development as Rahner understands it, theological discussion, the influence of the Spirit and the role of church authority. The discussion of a possible definition of Mary as co-redemptrix and the controversy over the ordination of women are cited as concrete examples of issues of development facing the (...) church today. Rahner’s increasing awareness of the context of irreducible plurality that marks the world-church of today and tomorrow led him to the increasing conviction in the later works that the Spirit is leading the church to a deepening understanding of the central mysteries of faith rather than to a further proliferation of defined dogmas. In the later works especially, he encouraged an attitude of open discussion that reflects the confidence that the Spirit is with the whole church as it struggles to express its faith in ever changing contexts. The paper concludes that Rahner’s understanding of the complex balance of elements involved in authentic doctrinal development continues to offer valuable insight to today’s discussions. (shrink)
